Medication Policy

As a service to parents, we will administer prescription and non-prescription medication to children upon request. We are, however, bound by strict state guidelines. In order for us to administer medication to your child, the following rules apply:

  • Parents must fully complete and sign a Medication Administration Request for each medication each day.
  • Medication must be in original bottle
  • If prescription medication , it must be prescribed for the child (not a sibling)
  • Instructions given by parent must match those on the bottle for both prescription and non-prescription medication
  • If bottle says four times a day, we can only give it every six hours
  • If bottle says 1tsp, we cannot give 3/4tsp even if parents do so at home
  • If bottle says ‘do not administer to a child under two except with advice of physician,’ a physician’s statement is required for us to administer the medication to a child under age two
  • Medication cannot be expired
  • Medication must be brought to center daily
  • Medication must be stored in locked box-Primary House stores medication in locked boxes whether stored in refrigerator or at room temperature.

Medication Administration Requests can be obtained at the front desk by Sign In/Out Sheet. They include the following: Child’s full name, Medication, Amount, Time, and Signature in the parent section. The gray section is for staff use. The information is repeated in this section and will be completed when the medication is administered. Parents will be given a copy of completed Medication Administration Request when they pick up child.

Non-prescription medication will be labeled with the child’s name and date when it is brought in.

Non-prescription diaper cream can be kept in diaper bag and does not need to be signed in, but prescription cream must be signed in.

Sunscreen can be signed in, labeled, and left at the center. A daily request is not necessary. The one-time Medication Administration Request when sunscreen is brought to center is all that is necessary.
